Building and Asset Action Request

Basic Information

This form is to be used to report a problem with a music facility or to request services related to current inventory and property (e.g., sending unwanted furniture to university surplus).

In EMERGENCY SITUATIONS or circumstance requiring immediate attention, do not use this form until you have reported the incident to the proper authorities.

For building emergencies (e.g., fire) call 911.

For situations requiring immediate assistance (e.g., burst pipe, flooding, leaking roof, broken exterior door) contact the Office of Facility Services (emergencies) at (225) 578-2327.

For request cleaning of potentially hazardous situations (e.g., spills in the hallway, broken glass), contact the Office of Facility Services (front desk) at (225) 578-6964.

For problems with heating, ventilation, and air conditioning, contact the Office of Facility Services (routine maintenance) at (225) 578-3186.

Once the situation has been reported and under control, please proceed with completing this form to notify the building coordinator(s) of the situation.

Facility Request

Property or Inventory Requests

When requesting that an item be sent to university surplus, THE ITEMS MUST NOT BE PLACED IN THE HALLWAY, as this creates a potential fire safety hazard. Once this request has been submitted and processed, the appropriate staff member will contact you with instructions for the removal of the item. Placing the item in the hallway may result in a delay in this request being processed.
